How to Prepare for a Hysterosalpingogram (HSG) Procedure

An HSG (Hysterosalpingogram) is an X-ray examination of the uterus and fallopian tubes that uses a specialized low-dose X-ray technique called fluoroscopy and a contrast material containing iodine, which is visible on the X-ray.

What is an HSG Test For?

The primary purpose of an HSG exam is to help your doctor diagnose the cause of subfertility or infertility, as well as recurrent miscarriages. This procedure primarily checks for blockages in the fallopian tubes and can also provide valuable information about the shape and structure of the uterine cavity, including the presence of any scarring.

When Should HSG Not Be Performed?

This procedure should be avoided if:

  • You are pregnant or think you may be pregnant.

  • You have a pelvic infection.

  • You have an allergy to iodine-based contrast material.

How to Prepare for an HSG Exam:

The HSG procedure is typically scheduled between 7-10 days after the start of your last menstrual period. Unlike other medical procedures, there are no dietary restrictions, so you can eat and drink as usual on the day of your test. Preparation for this procedure is minimal, but it is important to ask your doctor any questions beforehand to ensure you feel informed.

Taking ibuprofen a few hours before the procedure is recommended to help reduce any potential discomfort.

What to Expect During the Procedure:

The procedure usually takes 30-45 minutes. You will be positioned on the exam table similarly to a gynecologic exam—lying on your back with your knees bent and feet in footrests.

Here’s a step-by-step breakdown:

  1. The radiologist inserts a speculum into the vagina to view the cervix and then places a small catheter into the uterus.

  2. The speculum is removed, and contrast material is injected through the catheter while X-ray images are taken.

  3. You may feel discomfort during the insertion of the catheter or the injection of the contrast material, which can vary from mild to more intense pain. If needed, anesthesia can be requested.

After the HSG Procedure – What to Expect:

HSG is an outpatient procedure, meaning you can go home afterward and resume normal activities. You may experience light spotting and cramps for about a day after the exam.
